Mingary Care collects this information to assess your volunteer application, contact you about volunteering, support your safety, meet record-keeping obligations and manage our volunteer program. Your information is stored securely and only accessed by people who need it. Some information may be shared with relevant third parties where needed (e.g. Volunteering Sunshine Coast, Services Australia, screening providers, insurers or emergency services). You can ask to access or update your information at any time.

Volunteer declaration. By submitting this form I confirm the information provided is true and correct to the best of my knowledge; I understand this is an application for an unpaid volunteer role and does not create an employment relationship; I understand completing this form does not guarantee a placement; I understand Mingary Care may contact me about my application, induction, roster, role updates and safety information by phone, email or text; I agree to take part in any required induction, training and role-based checks before I start; if accepted, I agree to follow Mingary Care’s policies, procedures and safety instructions; I agree to treat confidential information with care and respect; and I agree to let Mingary Care know if my circumstances change in a way that may affect my ability to volunteer safely.

If accepted as a volunteer, I understand I must not attend a volunteer shift or carry out Mingary Care duties while affected by alcohol, drugs or medication in a way that could affect my safety, judgement, behaviour, or ability to safely perform my volunteer role.
